Dear Patients and Friends: We are writing to inform you of an exciting new partnership between the U-M School of Nursing Nurse Managed Health Centers and Packard Health. This alliance allows our nurse faculty to continue seeing and treating patients just as they have been, with the added benefit of operational support that will enable us to care for the community for years to come.

As a result of the decision to partner with well-established health practice, Packard Health, we will be relocating Community Family Practice and North Campus Family Practice on February 6, 2012 to Packard Health. Their main clinic location is 3174 Packard Road, Ann Arbor, Ml48108, and their Packard West clinic location is 501 North Maple Road, Ann Arbor, Ml 48103.

Starting in January, 2012, Elizabeth Lundy-Wiegers, MS, FNP-BC, currently our health care provider at Community Family Practice, will be joining the staff at Packard Health. She will be working at both the Packard West clinic and the Community Family clinic for the month of January, 2012, and permanently relocating to Packard Health starting February 6, 2012.

In the event you should chose not to continue your care with Packard Health, it is very important that you make arrangements as soon as possible to select a new care provider to make sure you receive uninterrupted medical care. Please contact your medical plan to obtain a list of providers in the area who are eligible to become your primary care provider. If you have coverage through Washtenaw Health Plan, your care must be received at Packard Health.

If you elect to receive your care with Packard Health or if you choose a different provider, an authorized release of records is required of you. Your medical records cannot be released without a written authorization from you. For your convenience, we are enclosing with this letter an authorization form for you to complete and return to the office once you have decided where you would like to receive care in the future.

In the event we do not receive guidance from you before February 6, 2012, your medical record will be securely transferred and stored at the University of Michigan Health System's Health Information Management Office. If you would like to request your records after February 6, 2012, please bring, email or fax the attached authorization form to the U-M Health Information Management Office. Addresses, phone numbers, and instructions are provided on the form.

Please visit www.packardhealth.org for directions and hours of operation. The main clinic is accessible via AATA route 5, and the Packard West clinic is accessible via routes 12 and 18. There is secure and ample parking at both locations.
